Once you decide to work with me we will meet for an initial consultation and discuss your main concerns and what you hope to achieve. It often takes several sessions to choose the type of treatment best suited to your needs. In addition, you may want to take a couple of sessions to see if I am the right “fit” for you. At the end of the consultation process, you will receive a verbal assessment and a recommendation for treatment.
I participate in a few health insurance plans as an in-network provider. If you see me out-of-network, confidentiality is assured. Many clients prefer this method because therapy can then be based on personal goals and work to be accomplished. This arrangement provides you with more control.
Confidentiality and Release of Information:
Confidentiality is taken very seriously. All evaluation and treatment information is confidential and can only be released to specified individuals or agencies with your written release. When you arrive at my office, you will be asked to sign the Notice of Privacy Practices (HIPPA) form.
I am required by law to report suspected child abuse or dependent adult or elder abuse to the appropriate authorities immediately. If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
If a client intends to harm him/herself, I will make every effort to enlist his/her cooperation in ensuring their safety. If he/she does not cooperate, I will take further measures, without their permission, that are provided to me by law.
I require a minimum of 24 hour notice for cancellation for all appointments. When you do not keep an appointment and do not contact me with 24 hour notice, it is a loss of appointment time that could be offered to another person and the full fee will be collected. More importantly, continuity in our work together builds momentum; and consistency is recommended to make treatment most effective.
